Job Title: Clinical Social Worker (LICSW/LCSW)

Location: Newton, MA Area

Overview: Join one of the Boston area's premier retirement communities as a Clinical Social Worker in their Skilled Nursing Center. This unique facility, with under 90 beds, is renowned for its exceptional clinical programs and individualized care plans. As a key member of the leadership team, you will play a crucial role in creating an outstanding experience for residents.

Key Responsibilities:

• Participate in discharge planning and develop and implement social care plans and resident assessments.
• Conduct private interviews with residents and families to complete psychosocial assessments, obtain social histories, and formulate social care plans.
• Assist in arranging transportation to other facilities when necessary.
• Provide information to residents and families about Medicare, Medicaid, and other financial assistance programs.
• Coordinate social service activities with other departments as needed.
• Evaluate social and family information to assist in determining plans for social treatment.
• Consult and interpret social, psychological, and emotional needs of residents to the resident care team.
• Obtain resources from community social, health, and welfare agencies to meet resident needs.

Qualifications:

• LICSW or LCSW
