Job Description

We are seeking compassionate and dedicated Care Managers to join our team. In this impactful role, you will play a crucial role in supporting individuals with I/DD by coordinating their care and advocating for their needs.

What you'll do:

Conduct comprehensive assessments and develop personalized care plans.
Coordinate a wide range of services, including medical, behavioral health, and social supports.
Advocate for individuals with I/DD to ensure they receive the care and support they need.
Build strong relationships with individuals, families, and healthcare providers.
Maintain accurate and up-to-date records.
Participate in team meetings, trainings, and supervision sessions.
What you'll bring:

A Bachelor's degree in a relevant field (e.g., social work, human services) or a Registered Nurse (RN) license with relevant experience.
Excellent communication, interpersonal, and organizational skills.
Strong analytical and problem-solving skills.
The ability to work independently and as part of a team.
A commitment to providing high-quality, person-centered care.
Bilingual proficiency in Korean is highly preferred.
