BAYADA at Inspira Home Health is seeking an Admissions Registered Nurse (RN) to join our team. This full-time position focuses on helping patients transition smoothly from hospital to home, working in collaboration with Inspira Health and our dedicated care team.

What Makes This Role Unique

As an Admissions Nurse, you’ll provide hands-on care in patients’ homes, while working collaboratively with onsite transitional care managers and hospital discharge planners to eliminate any gaps in patient care during their transition. Together, you’ll support the alignment of hospital discharge goals with home health outcomes, ensuring patients receive the highest quality care to meet their personal health goals.

What You’ll Do:

• Visit patients in their homes to assess their needs and develop a comprehensive and customized care plan.
• Collaborate remotely with our office and hospital-based management team to ensure smooth transitions from hospital to home.
• Educate and support patients and families, empowering them to succeed in their recovery journey.
• Monitor patient conditions, administer care (e.g., wound care, medication), and document all services provided using state-of-the-art technology.
• Serve as a trusted guide, bridging the gap between hospital discharge and home care success.

What You Bring to the Role:

• Current Registered Nurse (RN) license in New Jersey.
• A minimum of one year of recent verifiable clinical (medical/surgical) experience (home care and/or admissions experience preferred but not required).
• Strong communication and interpersonal skills for remote collaboration with hospital staff and patients.
• Ability to work independently, travel to your cases as assigned in the field, and managing your time and caseload effectively.
• Proficiency with technology, including electronic medical records (EMR).
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
