Job Description

PURPOSE OF POSITION

As a member of the ACT Team, the Staff Nurse is to meet all New York State Office of Mental Health (NYSOMH) standards for ACT Teams, as well as all Gotham Health East New York and H+H policies and procedures.

Areas Of Responsibilities

Staff Nurse provides nursing care for severe and persistent mentally ill patients in the community. These functions include data collection, monitoring and observation, implementation of treatments, therapies, patient education, provided to Ambulatory Care patients between the ages of 18-100.

She/he collaborates with the multidisciplinary mental health team in providing care to the patient, family and/or significant others in a manner which considers the psychosocial, spiritual and cultural factors that may impinge upon their perception of mental illness. Purpose will be to provide safe and therapeutic care to adults with mental health diagnoses.

Summary Of Duties And Responsibilities

  • Collect data for assessment. Participates in plan of care for ambulatory care patients considering age and special needs
  • Provide direct patient care services appropriate to age of patient
  • Use effective communication techniques appropriate to age and maturity level of patients
  • Demonstrate positive interpersonal relations in dealing with staff, patients and visitors to promote teamwork, efficiency and quality patient care.
  • Collaborate with interdisciplinary team to provide appropriate health care needs based on age and medical condition of the patient
  • Administer medications according to establish policy.
  • Assist in coordination of medication management for clients.
  • Run medication and or other health care education groups using educational interventions:
  • Complies with East New York’s facility and departmental policies for safety, quality improvement and infection control.
  • Initiate emergency interventions based on clinical manifestations (i.e., choking, aspiration, respiratory arrest and cardiac arrest)
  • Demonstrate safe use of equipment, safety precautions in all aspects of patient care activities
  • Identify and intervene appropriately in cases of known or suspected child, elder abuse, and domestic violence
  • Carry out rehabilitation and support functions including:
  • Follows East New York’s facility and departmental policy for referrals as indicated
  • Provide escort to patient
  • Provide crisis intervention
  • Provide consultation to families and family psycho-education groups
  • Perform case management duties including:
  • Perform duties of service coordinator, which include
  • Use evidence-based techniques in providing wellness management. Assess the patient’s strengths and needs related to their understanding of their mental health problems, needs, treatment expectations for recovery and their effectiveness in coping with symptoms, managing stress, and forming/strengthening relationships.

  • Coping and problem-solving skills

  • Coping skills to manage stress
  • Using medications effectively

  • Integrating dual-recovery treatment with the task of other members
  • Act as liaison between patient and community-based organizations
  • Document and maintain contact with patient through home visits and or telephone calls
  • Assist patient to obtain entitlements (Medicaid, Medicare, and Social Security Benefits) and housing
  • Write service plans
  • Insuring immediate revisions to the service plan as the recipient’s needs change
  • Advocate for recipient’s rights and preference
  • connect information and skill learning with personal goals
  • Promote hope and positive expectation for success in achieving goals
  • Explore pros and cons of change (costs and benefits) and
  • Re-frame experiences in positive light.
  • Participate in unit/departmental Quality Improvement activities
  • Maintain a level of competency by attending continuing education programs and other activities consistent with area of clinical practice and the population served.
  • Provide patient/family education according to plan of care appropriate to age and readiness for learning and consistent with the cultural, ethnic and religious beliefs of the patient and family.
  • Adhere to organizational policy for annual education, annual assessment and in-service education
  • Document in accordance with East New York’s standard of practice and regulatory agencies’ guidelines.
  • Maintain security and confidentiality of information in accordance with Patient Rights Policies
  • Ensure and respect privacy and confidentiality while providing care.
  • Document nurse’s progress notes and all efforts to meet with patients. (All progress notes should be written within 24 hours of encounter with patient).
  • A minimum of 90% of individual visits will be in the community
  • Provide direct patient care in the community
  • Attend all clinical/staff meetings.
  • While any one staff member may have the primary responsibility to serve in a particular capacity, as the need arises, any one staff member can be asked to do or assist in completing a task within the limits of licensing, certification and skill requirements.

ACCOUNTABILITY

The Registered Nurse is accountable to the Head Nurse/Charge Nurse and through the Head Nurse/Charge Nurse to the Associate Director of Nursing.

Minimum Qualifications

  • A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Special Note

For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
