Job Description

A dedicated Special Education Case Manager is needed to support transition-age students in an innovative program focused on preparing young adults with moderate to severe disabilities for life beyond high school. This contract position is based in Chicago, IL, at an elementary school and serves students ages 1822 through a community-based Transition Program.

Key Responsibilities:

• Manage caseload for students enrolled in the 1822 Transition Program.
• Develop, implement, and monitor Individualized Education Programs (IEPs) centered on transition goals in employment, education/training, and independent living.
• Create and deliver individualized instructional plans aligned with each student's postsecondary objectives.
• Support the acquisition of functional, vocational, and life skills.
• Coordinate community-based learning experiences to enhance independence and job readiness.
• Collaborate closely with families, related service providers, job coaches, community partners, and adult service organizations.

This Transition Program emphasizes real-world learning, providing job training, employment opportunities, and community instruction tailored to students interests and strengths. The program fosters student-centered planning and collaboration to maximize independence and successful post-school outcomes.

Qualifications:

• Valid Illinois Professional Educator License (PEL) with a Special Education endorsement, including LBS1.
• Experience working with transition-age youth with moderate to severe disabilities is preferred.
