*Responsibilities*
* Supervise nursing staff and coordinate patient care activities.
* Develop and implement care plans tailored to individual patient needs.
* Ensure compliance with healthcare regulations and best practices.
* Oversee case management processes to optimize patient outcomes.
* Provide training and support to nursing staff on procedures such as catheterization and tube feeding.
* Maintain accurate patient records using electronic health record systems like Epic.
* Collaborate with interdisciplinary teams to enhance memory care services.
* Monitor patient progress and adjust care plans as necessary.

*Skills*
* Strong knowledge of physiology and nursing principles.
* Proficiency in family planning practices.
* Experience in dementia care and memory care strategies.
* Ability to perform catheterization and manage tube feeding procedures.
* Familiarity with case management processes and documentation standards.
* Understanding of HIPAA regulations to ensure patient confidentiality.
* Excellent communication skills for effective collaboration with patients, families, and healthcare teams.
