Job Summary: The Case Manager, RN - Palliative is responsible for the assessment, planning, implementation, and evaluation of palliative patient referrals within the MWHC Emergency Departments. The incumbent in this position will collaborate with providers to ensure well-coordinated, multi-disciplinary programs of care. This position will also allow for increased coordination of care, streamlining end of life processes, reduction of re-admission rates, and improved attention to goals of care.Essential Functions & Responsibilities:
