Avera is currently seeking Registered Nurses (RN) looking to deliver the best possible nursing care in Avera’s patient-focused settings.

Job Summary

The Hospital@Home model makes acute hospital-level care available to adults and particularly older adults where they live, rather than in a traditional hospital setting. This alternative is needed because hospitals are not always the right environment for many patients. A substantial body of literature points to the hazards of hospitalization, notably for people 65+, including delirium, pneumonia, falls, polypharmacy, pressure sores, and infections. Persons 80 years and older and those with cognitive impairment are at particular risk for death during hospitalization. .

This position Registered Nurse Case managers provide nursing care, care coordination, lab draws, nebulizer treatments and other cares as needed for patients during their acute stay. 

Education and/or Experience

• Graduate from a CCNE or NLN accredited nursing program is strongly preferred. Graduate from an approved nursing program is acceptable.
• Bachelor’s degree in nursing preferred.
• Minimum 2 years related experience and/or training; or equivalent combination of education and experience preferred.

Licensure/Certification/Registration

• License as a Registered Nurse (RN) in South Dakota and/or meeting the requirements of Multi-state licensure is required.
• Licensed as a Registered Nurse (RN) in the State of Minnesota may be required depending on the service area. If required, must obtain within 90 days of being placed in the position or of the requirement.
• This position requires a valid driver’s license and that the employee is insurable by the Avera Health automobile liability insurance carrier.

Work Schedule

• Varied Shifts, e/3rd wknd/hol, occas call; 72hrs/2wks

Pay

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.  

• $29.50 - $44.25 per hour
