OVERALL RESPONSIBILITY:

Case Management and Care Coordination

1. An Initial Assessment that assesses the client's functional limitations, resource availability, and identified community supports to determine the case management interventions delivered within ESH's Outpatient Clinic.
2. Care Coordination shall consist of the following interventions as deemed appropriate for the client's needs:
   1. Assistance in the development and implementation of a plan for accessing benefits and entitlements to include the completion of a Medicaid application (if not already enrolled) and Social Security Insurance (SSI) application when appropriate.
      1. The program case management will work with ESH's fiscal unit to assist in the verification of any active medical coverage and eligibility for state programs to include Medicaid, SSI, and any state waiver services
   2. Interacts with the primary community provider for the client on a regular basis (at least monthly and more often if a situation arises in which additional case management and psychosocial supports are needed) to determine that the client is receiving appropriate psychiatric and psychosocial services in the community and to discuss ongoing progress and needs of the client
   3. Assistance in locating and effectively utilizing all necessary community services in the medical, social, legal and behavioral health areas and ensuring all services are coordinated
   4. Assistance in coordinating appointments to receive the Slater Method training when appropriate
   5. Assistance in coordinating court ordered treatment when appropriate
   6. Coordination with other service providers to monitor the client's health status, medical conditions, and treatment adherence
      1. In the instance in which the client is not receiving mental health or substance abuse services, the client will be referred to a Community Mental Health Center or Substance Use provider as well as provide

ongoing consultation with the provider to ensuring the client is receiving adequate services to meet their identified needs

1. 1. 1. In the instance in which the client is diagnosed with an Intellectual or Developmental Disability and is not active with the Division of Developmental Disabilities, the case manager will assist the client with the application process and serve as a liaison to the Division.
   2. The clinician shall assist the client in devising a transportation plan to ensure compliance with forensic and community appointments
2. Provide counseling services as needed
3. Provide referrals for any services determined to be a need
4. Provide clinical supports in assisting client and family, when appropriate, with personal or environmental difficulties which are centered around the illness, or which interfere with obtaining maximum benefits from medical and psychiatric care
5. Provide updated reports to BHDDH legal department as needed
6. Complete regular contact notes for patient record
7. Complete appropriate discharge plan when client has terminated forensic involvement
8. Enhance continuity of care, facilitate adjustment to the community and to ensure that necessary community services are provided
9. Advocate for least restrictive environment to receive identified service needs
10. Reviews all cases with case manager and supervisors to assure quality care is being delivered.
11. Assists with program policy, development and management of the program

Requirements :

• Licensed or eligible for LCSW, LICSW, LMHC
• Master degree in social work or related field
• Minimum 2 yrs. experience required
• Previous experience in mental health services and/or forensic population

Desired: CDP, QMHP

35 hours/week. Some schedule flexibility required.

Master's Degree in social work or closely related field is required.

Position is for 1 year - possible to be extended.
