Job Description

The Clinical Informatics Pharmacist will enhance medication management through data analysis and informatics solutions. This hybrid role involves collaboration with healthcare teams to optimize patient outcomes in Lebanon, NH. Responsibilities:

Analyze and interpret clinical data to improve medication safety and efficacy
Collaborate with healthcare professionals to implement informatics solutions
Provide education and training on clinical informatics tools
Monitor and evaluate medication use and outcomes
Participate in quality improvement initiatives

Requirements:

PharmD or equivalent degree
Active pharmacist license in NH
Experience in clinical informatics preferred
