Day to day as a Clinic Health CoachAs a Health Coach at Guttenberg Municipal Hospital & Clinics, you will play a pivotal role in delivering exceptional care through collaboration with physicians, providers, and staff. Your responsibilities will include coordinating care for targeted patients within the community, ensuring safety, best practices, and maintaining high-quality standards across the healthcare continuum. You will actively engage in self-management support and disease registry activities, contributing to the quality improvement processes at our family practice clinic. Moreover, you will perform point-of-care lab testing, administer medications and immunizations, and facilitate communication, referrals, and authorizations within the care team.
With shifts available at any of our three Cornerstone Family Practice clinics, this position offers flexibility and the opportunity to foster meaningful relationships with patients, ultimately enhancing their health and well-being.
What you need to be successfulTo thrive as a Health Coach at Guttenberg Municipal Hospital & Clinics, candidates must possess a blend of clinical expertise and interpersonal skills. A graduate of an accredited LPN or RN program, you must hold a current Iowa nursing license (RN or LPN) and be certified in Basic Life Saving. While a Health Coaching Certification is preferred, strong communication and problem-solving abilities are essential for effective collaboration with healthcare providers and patients alike. Proficiency in relevant software tools will aid in care coordination and documentation, enhancing your efficiency in managing patient records and reports. A customer-centric mindset, combined with empathy and a commitment to excellence, will empower you to provide exceptional support to targeted patient populations.
